My endo resto story: this deep secondary caries allow the bacterial invasion to the pulp and periapical on maxillary right second premolar

A patient came with a spontaneous pain on her maxillary right premolar. Had composite resin filling for more than 7 months but the symptoms still occurs and even the food retention always made her stress. After taking a proper informed consent, i started my dental treatment.

the diagnosis was chronic apical periodontitis with pulp necrosis. the bacterial invasion might be started from the leakage of previous composite resin filling. following the removal of previous composite resin filling, i did the gingivectomy and rewalling with composite resin to build up the distal wall. why?? in order to create a 4-walled cavity access prior to endodontic treatment.

shaping was done with i3Gold files from denjoy flooded with copious amount of sodium hypochlorite 5,24%. i love this concentration due to its ability to fast removing the organic substances inside root canals. agitation of irrigant was done with ultrasonic approach.

coronal seal post obturation was filled with low shrinkage stress flowable composite resin, core build up with packable bulkfill, and final restoration with packable composite resin. final x-ray evaluation showed an intact margin and hermetic filling from apical (gutta percha) to coronal (composite resin).
