Endo Resto Story on maxillary left first molar (4 canals with MB2) with previous overhanged and improper glass ionomer filling leads to pulpal and periapical infection

a patient came with a spontaneous pain on his upper left first molar which had been filled by another operator for more than 2 years ago. pain on percussion, slight radiolucency at the periapex, and tooth still vital. after taking a proper infomed consent, i started my dental treatment.

the diagnosis was asymptomatic apical periodontitis with pulp necrosis. in order to create a 4-walled cavity access, i did build up the mesial wall with composite resin. i thought, at first, that the orifices were only three, but then i knew i got bonuses that i found four! hehe

shaping the root canals was done with i3Gold files (Denjoy) flooded with sodium hypochlorite 5,24% (very great to kill all the microbes). agitation of irrigants inside root canal with ultrasonic approach (irrisafe from satelec).

final restoration was composite resin, and i used fiber-reinforced composite resin (EverX from GC) as core build up material.

My Endo Resto (restodontic) revision treatment on maxillary left first molar with 4 canals

A patient came with a persistent pain on his upper molar which had endodontically treated by another operator and had broken amalgam filling. The diagnosis was chronic apical periodontitis with improper endodontic treatment. after taking a proper informed consent, i started my dental treatment.

everytime i did my revision of previous root canal treatment, i didn’t (or very rare) to use specific file for gutta percha removal. i am only using a rotary file that also used for shaping the root canal wall. in this case, i used i3Gold files from Denjoy.

like usual, coronal seal was placed after obturation. i used low shrinkage stress flowable composite resin. and as my final restoration, i used composite resin.
