Endodontic: From Rebuild the missing wall to Endodontic Treatment

My rule in endodontic treatment on missing one or more lateral/proximal walls is to rebuild the wall. Not only to optimize the irrigation protocol, this will provide a space for temporary filling.

This patient represent the improper placement of temporary filling. Even, i dont know what the black-colored cotton pellet is. Is it arsenic paste? What if it is leaked to the oral environment?

After taking proper informed consent, i started my endodontic treatment. I placed rubber dam (blossom Rubberdam sheet and Sanctuary rubberdam clamp) prior to rebuild up the missing wall as my first treatment. The distal wall was created with bulkfill composite (filtek bulkfill – 3M) with OmniMatrix (ultradent). Cleaning and shaping with ProtaperNext (dentspy), irrigation with Natrium Hypochlorite (NaClO) 5,24%. Obturation with Guttapercha, then sealed with low shrinkage stress flowable composite (Filtek bulkfill flowable – 3M).
Xray evaluation with VistaRay 7.1 (Durr) showed intact margin between composite resin, dentin, and gutta percha.

Then, this tooth ready for coronal restoration. So.. stay tuned!

Post Endodontic Direct Restoration: 4 weeks later

After waiting several weeks to see the result of my one visit endodontic treatment (i also have shared the case), my patient came back to do restorative procedure followed with rehabilitative ones by placing implants.
To differentiate with my previous treatment, i changed the rubberdam color from dark blue (by cerkamed rubberdam) to sky blue (by nictone rubberdam).

Last time, i put low shrinkage stress flowable composite (filtek bulkfill flow from 3M) as coronal seal.
This time, I used greatercurve matrix to build up the disto-lingual-mesial wall prior to placement of first increment of fibre reinforced packable composite, ever-x from GC. Then, i used packable composite, estelite from tokuyama, as final coronal layer.
Radiographic evaluation with VistaRay 7.1 (Durr) showed intact margin between composite resim and dentin, also homogeneous body of restoration. This will provide a well distributed occlusal force in longevity of restoration.
