Case 10: Class II in free contact point on proximal

Assalammualaikum wr wb.. Good Morning!

Today, fifty minutes past 7 am, i am going to give a new story on SekilasKonservasiGigi.Com, conservative dentistry at a glance.. a mini story of CeritaPasienRio.Com,  🙂

Yesterday, January 26th 2015, I got a referral from my fellow dentist to fix a broken EMAX crown on tooth 15 and a heavily broken restoration plus secondary caries on tooth 14.

the previous operator had done a root canal treatment, followed by placing a fiber post + Composite core prior to EMAX Crown (I got the X-ray). But, now, let’s not focus on tooth 15, we will do it later.. because i want to focus on tooth 14. Yes, a broken restoration, why?? because, we can not do a proper crown restoration while there is no good contact point in proximal due to a broken proximal wall in tooth 14. 🙂

Some believes that sectional can not be used in free contact area because it will make the proximal wall bigger than expected or the ring will not fit in those area. but, in certain cases i think it is a mandatory.

So, C’mon see the case…

Picture Information:

1. initial view form occlusal, we can see the broken EMAX Crown on tooth 15 and a broken composite resin restoration plus secondary caries beneath it on tooth 14.

2. initial lateral view of both premolar teeth. see the black line across gingival wall as the edge of restoration margin. this is the location where the carious lesion begin.

3. finished cavity preparation on tooth 14.

4. placement of sectional matrix and adhesive application.

5. creating a class I cavity from class II cavity by building up the missing wall.

6. finished restoration.

yup, i only used a two body shade on this case. A3 and A3,5 from Z250 (3M ESPE).

alhamdulillaah.. the case was done, and the story still continues..

dentistry is too complex to do it alone, Let’s do it together.. 🙂

Case 9: A Perforation To Periodontal Tissue

Assalammualaikum wr wb… Good Morning!!

Finally we meet again in here SekilasKonservasiGigi.Com, conservative dentistry at a glance.. a mini story of CeritaPasienRio.Com,  🙂

can you on perforation on periodontal tissue in a root canal treatment?? Yup, i think that you might experienced on seeing that case before.. but now is different, the perforation was being obturated.. this is the first time i experience this, and a question popped up in my mind “what was the pain feels like? did the previous dentist used local anesthesia while shaping the “periodontal bone”? or did not he/she see blood coming from the perforation site?”

You can see the tip of Gutta percha was not in a right direction and also touch the base of the sinus which made the tip a little bit bend.

Picture Information:

1. First view of tooth 15, the patient feels a spontaneous pain, pain while chewing or eating, and feels that something “hard” touch the area beneath the eyes (while the finger tip touch the area of sinus)

2. the original radiographic pictures showed us the obturation in the perforation site. We can see the tip of GP is bend might be due to touch the base of the sinus. the gutta perca is as long as the tooth.

3. when i opened the cavity, i found the coronal tip of GP point.

4. then i seek for the original orifices, and there they are!

5. (explanation)

6. I was able to pull out the GP point.

7. the GP point, i think it is F3 GP point.

 

alhamdulillaah.. the case was done, and the story still continues..

my advice, When You feel not sure with the case you faced, please contact the nearest endodontist.
