Sometimes, we found out a patient with a good-newly-cemented crown but had a persistent pain. And sometimes, the diagnosis is confusing. Thanks to radiographic image on giving the unseen image from our eyes..

Yes.. i got this case a referral from my friend, a dentist, who found out that a problem of persistent pain was from unfinished root canal treatment.
From my patient explanation, this RCT was done around less than 1 year ago, and the crown was made in 6 to 8 months ago. Xray image showed an under obturation and a radiolucent at the apical.

After taking a proper informed consent, i started the dental treatment.. i placed Rubberdam (blossom sheet and sanctuary clamp).
I opened the access through the crown from palatal. Cleaning and shaping was done with protaper next (dentsply) and sodium hypochlorite 5,24% also with EDTA.
I have no idea on the obturation material used by previous operator. It was grey and thick. I assume it was not a guttapercha nor plastic.
Then.. I Agitated with Endoactivator (dentsply). After obturation was completed, i seal the coronal portion with low shrinkage stress flowable composite, Filtek Bulkfill (3M). Xray evaluation with Vistaray 7.1 (Durr) showed an intact margin of guttapercha, composite resin, and dentin. Also a well manage ledge at the root canal.
