Apical Periodontitis and Traumatic Occlusion

My best friend, an orthodontist, sent me a patient with a discoloration on tooth 41 and apical periodontitis on tooth 42 due to traumatic occlusion. tooth 41 had been endodontically treated by another endodontist about 3 or 4 years ago (year 2012-2013). this tooth 42 was non vital, tenderness on percussion, and showing a craze lines defects, the incisal also showing fracture defect. Radiographic examination showed a good endodontic treatment on tooth 41 and radiolucencies in the apical third on tooth 42.

after taking a proper consent, I started my endodontic treatment on tooth 42 and planning on doing intracoronal bleaching on both 41 and 42. Root canal treatment was done with Protaper Next (Dentsply), Sodium Hypochlorite 5,24% agitated with Endoactivator (Dentsply), coronally sealed with low-shrinkage stress flowable composite Filtek Bulkfil (3M) after proper obturation.

this dental treatment still continues, and i will update it as soon as my patient came to me for the next appointment.

hope this treatment will last long.. 🙂
