1. 98% of mandibular first premolar had one root, 2% had two roots.

2. 70,6% had one canal, 27,8% had two canals, 1,2% had three canals.

3. The prevalence of c-shaped was 2,4%

4. 100% mandibular second premolar had one root.

5. 80,5% had one canal

6. 17,5% had two canals.

7. The prevalence of c-shaped canals was 2%

A while ago, a patient came to me, for this scenario we can call her Nami. 🙂
Nami came with a referral from my friend, dentist, due to the inability to find the right canal(s). The first condition showed an overhanged temporary filling and a inflammed proximal gingiva. From the xray, i saw two canals. For me, i think the problem lies on the access preparation.

After taking a proper informed consent, i started my dental treatment. First, I cautered the marginal gingiva, then i placed the rubberdam (cerkamed rubberdam sheet and sanctuary rubberdam clamp). Followed with creating/rebuilding the distal wall with bulkfill composite (filtek bulkfill – 3M)

Alhamdulillaah.. i found all the two orifices, one in the buccal, and the other lies on the lingual. Cleaning and shaping was done with Mtwo (VDW) with reciprocal motion (150 CW, 30 CCW) with Denjoy Endomotor. Coronally sealed with low shrinkage stress flowable composite resin (filtek bulkfill flow – 3M). Xray evaluation with VistaRay 7.1 (Durr) showed a fully obturated two canals, and intact margin on guttapercha-composite resin-dentin junction.

After a week post endodontic treatment (electro surgery, rebuild up the distal wall, root canal treatment, and internal bleaching), now Nami feels better. She can chew without any pain, all the previous sympthoms were gone, and ready to do restorative procedure.

After placing rubber dam (nictone rubberdam sheet, sanctuary rubberdam clamp), i filled the cavity with packable composite resin, Ceram-X one (dentsply), single shade. Then, polished it with soflex (3M) and enhance (dentsply). Xray evaluation with VistaRay 7.1 (Durr) showed an intact margin between composite resin, gutta percha, and tooth structure. Also, a homogeneous body of restoration. This condition will provide the longevity of the restoration.
