Sometimes, I also updated my cases at Line or Linkedin. 🙂 this topic will focus only on conservative dentistry cases such as restorative dentistry and endodontic or root canal treatment.

Compared with resin (4.7%) or porcelain (0.9%), the use of nonbonded inlay restoration materials such as gold (20.5%) or amalgam (18.7%) increased the occurrence of longitudinal tooth fractures (Deog-gyu Seo et al). As seen on this case, in direct restoration, amalgam gave the most effect on causing crack tooth.

Here, an upper right thid molar had been filled with amalgam for almost 5 years. The amalgam still exist but the tooth might be not. Crack lines occur from mesial to distal, adjacent to amalgam filling. The patient experienced hypersensitivity.

After taking a proper informed consent, i started treating the patient. Isolation is a must on removing amalgam. I put rubberdam (hy friedy rubberdam clamp and blossomrubberdam sheet) prior to amalgam removal. Then, i put low shrinkage stress packable composite resin, filtek bulkfill (3M) to restore the tooth. Finishing and polishing done with soflex (3M) and enhance (dentsply).

For me, xray evaluation is mandatory, i take it with Vistaray 7.1 (durr) to evaluate my restoration. It was beautifully intact to tooth structure and the body of restoration was homogeneously filled.
