Class II Restoration with Tunnel Preparation

Tunnel (ˈtənl)
an artificial underground passage, especially one built through a hill or under a building, road, or river.

While in here, tunnel preparation means a caries removal on class II cavity without removing a marginal ridge. This will create a tunnel between occlusal and mesial wall.

Let’s call my patient in this case as Risa. Risa felt discomfort with her upper right first molar due to two things. First, the black appearance of amalgam filling. Second, the food impaction in the proximal area. Risa is 35 years old, and this amalgam had been there since she was 25. Hmm.. i think, amalgam existence should be finished on that day i treat the patient, and replaced with composite resin.

After taking a proper informed consent, i placed rubber dam (blossom rubberdam sheet and hu friedy rubber dam clamp). Then, I Removed the amalgam, prepared the cavity with tunnel technique, placed the sectional matrix band (Triodent), followed with the placement of BulkFill composite (Filtek BulkFill – 3M). The restoration was polished with enhance (dentsply) and soflex (3M) at the proximal area. Finally, xray evaluation with VistaRay 7.1 (Durr) showed that there is an intact margin between composite resin and tooth structure, also a homogeneous body of restoration.

Alhamdulillaah.. Risa loved the new filling, although the color didn’t fully match. But for the long term function, it will give a predictable result.
