Many problem can occur from an overhanged dental filling, such as food retention which leads to cervical plaque accumulation prior to marginal periodontitis. Also, leaked dental filling may initiate a secondary caries prior to pulpal infection. What happen when those condition appear on one tooth?

Let’s call my patient name as Sissi. My friend, a dentist, sent her to me to solve a problem on upper right first and second premolar. I already did root canal retreatment on the second premolar, and a few days later i did the case on the first premolar.
The tooth still vital, radiographic image showed a radiolucencies (void) under composite resin. After taking a proper informed consent, i started my dental treatment.

At first, i put rubber dam (nictone rubber dam and hu friedy rubber dam clamp), followed with remove all the old filling, and matrix placement (greatercurve matrix). Then, i place bulkfill composite (filtek bulkfill from 3M) up to 2 mm from the coronal area, then i placed universal/packable composite (ceram-X-one) as coronal layer. Polishing was done with soflex (3M) and enhance (dentsply). Final xray evaluation with Vistaray 7.1 (durr) showed an intact margin between composite resin and tooth structure, plus a homogeneous body of restoration.
