Gingival and localized alveolar bone necrosis related to the leak of arsenic-contained paste in a necrosis tooth.

Got a referral from my friend, a dentist, who found out a leak of arsenic-contained paste to the periodontal tissue (done by previous operator) in a tooth need for endodontic treatment. This patient felt discomfort for this condition. And seek for better dental treatment.

After taking a proper informed consent, i started my dental treatment as follows:
1. Rubberdam: hu friedy rubberdam clamp, blossom rubberdam sheet (pre-endo procedure) and nictone rubberdam sheet (endo procedure)
2. Rewalling with composite resin filtek z350XT, with guidance from greatercurve matrix.
3. Polishing the composite resin with Soflex (3M) and enhance (dentsply)
4. Change the rubber dam fron multiple teeth to single tooth isolation.
5. Shaping with Mtwo files (VDW) with clockwise reciprocal motion and rotary motion with Denjoy Imate2C endomotor.
6. Cleaning with Sodium Hypochlorite 5,24% (generic), EDTA 18% sol (ultradent)
7. Obturation with Guttapercha and endorez system (ultradent)
8. Coronal seal with low shrinkage stress flowable composite: Filtek Bulkfill Flowable (3M)
9. Xray examination with Vistaray 7.1 (Durr) showed proper obturation, intact marginal integrity between composite resin – guttapercha – dentin, and good coronal seal.
10. Curretage done on the marginal gingiva to provide bleeding and healing (picture not shown)
